Treasurer Forms

Tere you will find the common financial forms that the King PTO uses.  Special thanks to the PTO Today for their work on the forms that we use.

Deposit Notice:

This form should be used to tally your deposit before sending to the Treasurer.  Deposits can not be accepted without the Deposit Notice form.

Reimbursement Request:

This form should be used when you are seeking to be reimbursed for previously approved personal expenses.

Check Request:

This form is used to request the PTO Treasurer to write a check for payment of product or services rendered to the PTO.

Cash Box Request:

This form is required when requesting a cash box for a PTO event.  Please note that this form should be filled out and submitted at least one week before the event.
